HOW DO I... REGISTER WITH THE PRACTICE? All new patients over 16 years of age are offered a health check-up as part of the registration procedure. This is carried out by our practice nurse or healthcare assistant and provides us with a record of your health needs and promotes early detection of illness. The purpose is to determine how we can best help you. Please bring the completed questionnaire and urine sample for testing at this check. In order to maintain continuity of treatment we encourage our patients to remain with the same doctor. Nevertheless, patients do have a right to express a preference of practitioner but, whilst the practice will endeavour to comply with this, it might not always be possible. |
